Fluorescence tech debut at Medical College Hospital Kolkata thyroid surgery on 38-yr-old man – ET HealthWorld
KOLKATA: Doctors at Medical College and Hospital (MCH) used the latest fluorescence technology for bilateral neck exploration and removal of parathyroid glands on a 38-year-old man. This is perhaps the first time such a technology has been used for this procedure in Kolkata. The patient is recovering well.
Bablu Mandal of Burdwan’s Salanpur was suffering from recurrent kidney stones. Test revealed high calcium levels. Doctors suspected hyperparathyroidism which is hyper functioning of one or more parathyroid glands. Further blood tests found raised parathyroid hormone in the blood. Normally humans have four parathyroid glands in their neck, sometimes in the chest cavity as well and the number can vary. When only one gland is affected, it can be removed through a targeted procedure called parathyroidectomy. However, if more than one gland is affected and none of them are localised comprehensively then both sides of the neck are surgically explored to look for enlarged glands.
In this patient, different images pointed to different glands being pathological and enlarged glands were also less than 1cm in dimension. “In such a situation, trying to look for the pathological glands and finding them by blind surgical exploration of the neck generally would amount to trying to find a needle in a haystack,” said Dhritiman Maitra, associate Professor and in-charge of breast and endocrine surgery at MCH.
The team, comprising Maitra, Shatakrotu Barman, Subhrajit Nath and Kaustubh Das went for fluorescence technology using indocyanine green dye and near-infrared camera to aid the detection of enlarged glands with positive fluorescence and their subsequent removal.
The procedure was backed by the anaesthesia team that included Koel Mitra Roy and Anindita Ghosh Biswas. The expensive camera setup was provided by manufacturer Stryker for free . The patient is being taken care by medicos Partha Pratim Chakraborty, Rameez, Neeti Agarwal and Animesh Maity from the hospital’s endocrinology department.
